This request form must be submitted at least 3 weeks prior to the Desired Air Date.
(The Desired Air Date is the Saturday date of the weekend in which you desire your story to be
aired during church services. Today is 5/9/08, so you must enter a Desired 1st Air Date as
Saturday 5/24/08 or any Saturday date after that one.)

Form Instructions: Please enter your Park AveNEWS Request in the online form
provided below. All fields marked with a red asterisk (*) are required fields.

entries that you have made. Any portion or all of the data that is entered may be
used in the actual Park AveNEWS production (except for your email address,
